To qualify for a Medical Cannabis Registry Card<\/a>, you must be a resident of Illinois diagnosed with one or more of the state\u2019s qualifying conditions (listed below). The following is required to apply for a card:<\/p>\n\n\n\n Minors (residents under 18 years old) seeking approval for medical cannabis must provide certification from two healthcare professionals. They must also choose a designated caregiver (up to three may be chosen).<\/p>\n\n\n\n If you\u2019ve been diagnosed with a terminal illness<\/a> with a life expectancy of six months or less, you may apply for a medical cannabis card valid for a six-month period. The application fee will be waived.<\/p>\n\n\n\n If your condition isn\u2019t listed as a qualifying condition and you believe you should qualify, you may petition the Illinois Department of Public Health<\/a> to add conditions to those listed<\/p>\n\n\n\nMinor patients<\/h3>\n\n\n\n
