The Rhode Island Department of Health requires in-person appointments to receive your medical marijuana certification.
After you book your appointment, you should log into your patient portal to complete your medical intake questionnaire to help the clinic and provider expedite your appointment.
Patient Requirements
1. Valid state photo ID with current address (passports may be used in conjunction with utility bill with proof of address)
2. Must have a qualifying condition 3. Medical documents pertaining to your qualifying condition will expedite your approval. If you need assistance obtaining documents we can help!
